The complimentary introductory benefit is limited to one eligible service per employee, up to a $800 retail value, and is available to currently employed team members of participating fertility/IVF practices. Identification or employment verification may be required.
The ongoing preferred professional rate provides 25% off eligible regularly priced MEND services. The benefit is personal to the employee and may not be transferred, exchanged for cash, combined with other promotions, applied retroactively, or used toward gift cards.
Medical services, laboratory testing, prescription medications, compounded medications, peptides, certain IV ingredients or upgrades, medical aesthetics, retail products, packages, memberships, third-party services, and other designated services may be excluded or subject to separate pricing.
All clinical and medical services remain subject to medical appropriateness, consultation, screening, informed consent, provider approval, and applicable laws and professional standards.
